What we need from you:
1. Fill in the form given below.
2. Attach a picture of you and your pet together.
3. Attach a photocopy of following documents:

  • Duly endorsed adoption form from any one of the following animal welfare organisations: Animal Lovers League (ALL), Cat Welfare Society (CWS) or Society for the Prevention of Cruelty of Animals (SPCA)
  • An official receipt from Pick-A-Pet as proof of purchase

Note: NRIC/FIN is NOT required. For submission of adoption form, please conceal/cover your NRIC/FIN.
